This form is only needed if you received the following initial grades: D-, D, D+, C-, P, or P+. 听If you received an 鈥淔鈥 in your initial completion of the course, you do not need to file this form.听If you received a letter grade of C or higher on your initial attempt, you may not be eligible to repeat a class with VA funding. 听Please check with Veteran and Military Affairs directly prior to completing this form.
